This role would work with their Community Health Worker teams at Bellevue Hospital (Manhattan) and Kings County Hospital (Brooklyn), specifically with the Special Populations programs including an initiative that supports people experiencing homelessness and people with a recent history of justice-involvement, who have complex care needs. The Community Health Workers also support a street medicine outreach team that is connected to their Bellevue site.
